Who uses the A107
A parent who is the spouse, civil partner or partner of the person applying to adopt. The child's other birth parent, if their consent is needed, uses form A104.
When it is used
With a step-parent's application for an adoption order (form A58).
Before you start
- The child's full name and a copy of the child's birth certificate.
- The applicant's full name.
- The names and addresses of anyone, or any adoption agency, who took part in the arrangements.
- An appointment with the officer who will witness your signature.
