Form C15 is a short supplement for an application for contact with a child who is in the care of a local authority. It asks how you are related to the child and what order you are asking for, and why.

Who uses the C15

A parent or guardian of a child in care, or someone who held a residence order or had care of the child through an order in force immediately before the care order was made.

When it is used

When you apply to the court about contact with a child who is subject to a care order, under section 34(2) and (3) of the Children Act 1989.

Before you start

  • The full names of the children.
  • How you are related to them: parent, guardian, or holder of an order in force before the care order.
  • Any report or document you will rely on, with its date and author.

How to fill in the C15

You state whether you are a parent or guardian, held a residence order in force immediately before the care order, or had care of the children through an order in force immediately before it.

C15 is a supplement. The main application form is the C1, which says not to complete its own reasons section when a supplement is sent with it.
