Who uses the LOC028
The applicant prepares one sheet per exhibit. It is completed and signed when the statutory declaration (LOC024) is made, by the solicitor, commissioner for oaths or officer of the court who witnesses it.
When it is used
When you apply by post. GOV.UK says that if you apply online, the cover sheet is emailed to you to print and complete after you have applied.
