Form P is a pension inquiry form. It asks a pension scheme for the information needed before the court makes a pension sharing or pension attachment order.

Who uses the Form P
The scheme member fills in Part A and signs it. The pension scheme completes Parts B, C and D.
When it is used
At the First Appointment the court may direct a party with pension rights to file and serve it. In an agreed pension sharing case where Form A was not served on the scheme, the member requests Section C.
Before you start
- The name and address of the pension scheme and your reference number.
- Your name and address, and any solicitor's details.
- The address the completed form should go back to.
- A copy of any court order requiring the information.
How to fill in the Form P

Front page and Part A
The member gives scheme details, their own details, any solicitor's details and a return address. They sign to authorise the scheme to release the information.
Part B: valuation
The scheme confirms the cash equivalent valuation and how it was worked out. It also covers membership for the other party and any charges.
Part C: sharing information
The scheme answers questions needed before a pension sharing order, such as other orders affecting the pension and extra information it needs.
Part D: benefits
The scheme gives details of benefits, earliest retirement date and death benefits, and sends a scheme booklet.

Who fills in Form P?
The scheme member completes and signs Part A. The pension scheme completes the rest.
How long does a pension scheme have to reply?
The form says 3 months for a valuation, or 6 weeks if told it is for divorce proceedings. Other information is due within one month, or sooner if the court says.
Is Form P the same as the pension annex?
No. Form P gathers information. Forms P1 and P2 are annexes attached to the final order.
